Statewide Program
The Statewide Family Support Program offers financial assistance to families
statewide, however, does not provide service coordination like the
local programs do.
The Statewide Coordinator establishes eligibility for the program from a simple
application form including documentation of the disability. The coordinator
then works with the family over the phone to coordinate the purchase of services
or supports that the family needs. The services and supports may include but
are not limited to the following:
- Diapers
- Medications
- Formulas/ Special Foods
- Recreational Opportunities
- Special Clothing
- Adaptive Equipment
- Housing Modifications
- Respite Care
- Family Counseling
- Parent/Sibling Education
- Travel Expenses for Medical Care
- Vehicle Modification
The coordinator pursues other sources before the financial assistance is provided.
For example, existing programs would be accessed for home weatherization needs;
the local school district's responsibility would need to be ruled out before
assistance be provided that could be their responsibility.
