Program Description:
Family Support 360 is a progrma for people with disabilities. It also helps your families. The program will do "whatever it takes" to give you the ability to get the services you need. These services will make it easier for you to live in the community on your own. People who work for this program can help you find the services you need to do this.
The PLANS Project is part of another program. The other program is called Family Support 360. PLANS started with money that came from a grant from a federal department. This department is called the Administration on Developmental Disabilities (ADD). South Dakota got the grant (money to use for a project) in 2003. The goral of this project is to help people with disabilities to live and work in the community. The people get the chance to choose the community on their own. There are also staff people who work with this project. They help people get the supports they need to be successful.
