statewide family support 

The Statewide Family Support Program is designed to assist families who have a child or adult with a developmental disability. A flexible pool of limited funds is available for eligible families, regardless of income, to use to purchase services or supports that otherwise are not available.

The Statewide Coordinator establishes eligibility for the program from a simple application form that includes documentation of the individual's disability. Once eligibility has been established, the Statewide Coordinator will then work with the family over the phone to coordinate the purchase of services or supports that the family needs. The services and supports may include but are not limited to the following:

  • Incontinence supplies
  • Medications
  • Nutritional Supplements
  • Recreational Opportunities
  • Adaptive Equipment
  • Housing Modifications
  • Travel Expenses for Medical Care
  • Vehicle Modifications

The Coordinator pursues other sources before the financial assistance is provided. For example, the local school district's responsibility would need to be ruled out before assistance is provided that could be their responsibility.

The Statewide Family Support Program does not provide service coordination. The Local Family Support Program provides service coordination, a wide array of services, and access to the Family Support 360 Waiver.

AM I ELIGIBLE ANd HOW DO I APPLY?

  • Diagnosed developmental disability
  • Requires prolonged assistance as determined by the Department of Education for ages birth to three
  • There are no income guidelines
