The South Dakota Division of Rehabilitation Services is conducting a consumer satisfaction survey. The survey results will help us in analyzing consumer's satisfaction with:

• Functions performed by our agency;
• Vocational rehabilitation services provided to our consumers; and
• Employment outcomes.

The satisfaction survey is being mailed to a random sample of consumers in the rehabilitation process. The survey instrument consists of a post card with only six questions. Consumers can submit their response by returning the post card, call 1-800-265-9677 to verbally provide their results or submit the results on the internet.
