Dakota At Home

Dakota at Home provides free objective information, referral, and assistance to help individuals regardless of age, disability, income or resources identify and access public and private long-term services and supports available in South Dakota's communities.

Dakota at Home is a no wrong door where trained staff will help you identify the long term services and supports you need to continue living at home and in your community.  We will answer your questions and provide decision support through Options Planning.

Options Planning

Options Planning is a free service offered when you call Dakota at Home.  Information and decision support is available to individuals, caregivers and family members to help identify available long term services and supports.  Through Options Planning you can obtain education and knowledge to plan for tomorrow on how to meet long term care needs at home and in the community.

 

Dakota at Home Resource Directory

Dakota at Home Resource Directory provides you with access to the services and supports available from agencies, organizations and providers in South Dakota.  The Learning Library provides information on topics of interest including:  Social Security, Medicare, Medicaid, Veteran's Services, Housing, Legal Services, Caregivers, Emergency Preparedness, Health, Consumer Information and Rights, Domestic Violence, Assistive Technology, and Employment.

Make a Referral

If you know of an individual who may benefit from receiving services provided by the Division of Long Term Services and Supports, you may complete the Referral Form by providing information regarding the individual, his or her contact information and type of assistance needed. A Long Term Services and Supports Specialist will contact the individual in follow up to the referral.
