Independent Living Review Questions

Below is a list of the current IL Review questions that state office staff use in their review instrument during annual on-site reviews. These questions also reference the IL Federal Regulations that the review is based on. This is provided to you as a tool to ensure that you are following the federal regulations when opening an IL client case and providing quality IL services.

1.Case Service Record (CSR) contains statement of need?

2.CSR contains correct and adequate eligibility documentation? CFR 34 §364.51

a. Documentation of a presence of significant disability?

b. Documentation of substantial functional limitation?

c. Documentation that services will assist the individual to live more independently?

d. Eligibility certification dated and signed by IL Staff?

4.CSR contains Independent Living Plan (ILP) or Waiver? CFR 34 §364.52

5. Client and staff sign/date ILP? CFR 34 §364.52

6.Client sign/date Waiver? CFR 34 §364.52

7.CSR contains client goals and objectives? CFR 34 §364.52

8. ILP contains services to be provided? CFR 34 §364.52

9. ILP contains anticipated duration of each service? CFR 34 §364.52

10.ILP contains anticipated duration of entire service program? CFR 34 §364.52

11.CSR contains documentation that copy of ILP sent to client? CFR 34 §364.52

12a.Were HMAD services provided?

b. If yes, did the CSR contain verification of SSA disability or form 314 correctly completed?

13. CSR contains record of services provided? CFR 34 §364.53

14. Were services delivered in a timely manner?

15. CSR contains record of client’s goals achieved? (closed cases) CFR 34 §364.53

16. If CSR is inactive, is reason identified? (closed cases)

17. If CSR is inactive, does it contain inactive date? (closed cases)

18. If staff terminated services, was the client provided a written explanation of both the reason and CAP rights?

19. Do services billed reflect appropriate amount of time to provide services?

20. If the ILP was longer than 12 months, was the ILP reviewed annually? CFR 34 §364.52

21. For clients seeking employment, does the CRS provide documentation of coordination with the Vocational Rehabilitation Program? CFR 34 §364.52

22. For clients with intellectual impairments, does the CRS provide documentation of coordination with the Community Support Provider or other developmental disabilities program? CFR 34 §364.52

23. For students with disabilities, does the CRS provide documentation of coordination with the Special Education Programs? CFR 34 §364.52

24. Was the closure reason correct and appropriate?

25. Does the client also receive TAD services?

25a. If yes, was the signed Assurance Document in the CSR?
