Resource Directory Inclusion/Exclusion Policy
The Resource Directory is a free service that provides older adults, people with disabilities, their caregivers and the general public with access to agencies, organizations, and providers of in-home and community-based services and supports in South Dakota.

The purpose of the Inclusion/Exclusion Policy is to define the types of entities, i.e., agencies, organizations, and providers that are eligible for listing in the Resource Directory, an on-line information and resource database.
Please read the policy for inclusion requirements before submitting your information.
Request for Inclusion Form
Complete the Request for Inclusion Form to submit information on the services your entity provides for consideration by the Department of Human Services Division of Long Term Services and Supports to be included in the Resource Directory.
