Nursing Facility Consolidation Grants
Division line
The Department of Human Services Division of Long-Term Services and Supports invites entities interested in planning for the merger of  long-term care services in a nursing facility setting to create, and be delegated as, a Regional Nursing facility to apply for funds to facilitate the planning and merger of facilities. 
 
Grant funds are currently available to Nursing Facilities or stakeholder/community groups working with a Nursing Facility in their community as follows:
 
Up to $100,000 per project for evaluation and planning grants;
Up to $1,000,000 or 50% of the total project cost, whichever is the lesser, for remodeling of a vacated nursing facility.  
 
The nursing facility must be a fully certified Nursing Facility enrolled with South Dakota Medicaid and interested in merging with one or more facilities within a 30-mile radius of their own facility.  The maximum award per combined proposal (planning and renovation) is $1,100,000 and is dependent on available funds. Projects cannot exceed two years.
